Proton - pump inhibitor - responsive esophageal

چکیده

Recent findings Several prospective series have reported consistent rates of PPI-REE (30–40%) in adults with suspected EoE. At baseline, PPI-REE and EoE have been shown to be indistinguishable upon clinical, endoscopic, and molecular characteristics [eotaxin-3, interleukin (IL-5), and IL-13 expression]. PPI therapy partially restores esophageal mucosal integrity in PPI-REE, but not in EoE. Anti-inflammatory effects of PPI therapy, independent of acid suppression, have been confirmed in EoE and gastroesophageal reflux disease cell cultures. PPI therapy in vivo downregulates Th2 cytokines in PPI-REE patients, in a similar fashion to that seen in steroid-responsive EoE.

Esophageal Eosinophilia Treated With Long-Duration Proton Pump Inhibitor Therapy

We present a case of proton pump inhibitor-responsive eosinophilic esophagitis (PPI-REE) in a patient with severe dysphagia and markedly elevated baseline esophageal eosinophilia that was previously deemed unresponsive to PPI. Upon reintroduction to PPI therapy with monthly endoscopy and dilation over the course of 4 months, the patient improved clinically and resolved her mucosal eosinophilia....

Proton pump inhibitor-responsive chronic cough without acid reflux: a case report

BACKGROUND Because 24-h esophageal pH monitoring is quite invasive, the diagnosis of gastroesophageal reflux disease (GERD)-associated cough has usually been made based merely on the clinical efficacy of treatment with proton pump inhibitor (PPI). CASE PRESENTATION We recently encountered two patients with PPI-responsive chronic non-productive cough for whom switching from bronchodilators and...

Two Cases of Esophageal Eosinophilia: Eosinophilic Esophagitis or Gastro-Esophageal Reflux Disease?

Eosinophilic esophagitis (EoE) and gastroesophageal reflux disease are among the major causes of isolated esophageal eosinophilia. Isolated esophageal eosinophilia meeting criteria for EoE may respond to proton pump inhibitor (PPI) treatment. This entity is termed proton pumps inhibitor responsive esophageal eosinophilia (PPI-REE). Gastro-esophageal reflux is thought to comprise a subgroup of p...

Food allergy and PPI-responsive esophageal eosinophilia

Background Eosinophilic esophagitis (EoE) is considered a chronic condition mediated by immune reaction to food and/or environmental allergens. Though first recognized decades ago, the characterization of EoE is ongoing and an important aspect of this process is the distinction between EoE and other forms of esophageal eosinophilia [1,2]. One group of patients exhibit marked esophageal eosinoph...

Proton pump inhibitor-responsive esophageal eosinophilia: a historical perspective on a novel and evolving entity.

Eosinophilic esophagitis (EoE) is an emerging chronic esophageal disease, first described in 1993, with a steadily increasing incidence and prevalence in western countries. Over the 80's and early 90's, dense esophageal eosinophilia was mostly associated gastroesophageal reflux disease (GERD). For the next 15 years, EoE and GERD were rigidly considered separate entities: Esophageal eosinophilia...
